Coverage Benefits
Issue Ages for Insured & Spouse 18-69.

Dependent Child under age 19 and unmarried or under age 23 if full-time student.

Group Policy and Rider Information

Continuation of Coverage	An employee may continue coverage after termination of employment or when he/she is no longer an eligible employee of the Policyholder so long as the group policy remains in force and renewal premiums are paid when due. Spouse may continue coverage if employee dies.
Benefit Reduction	Benefits reduce to 50% for any covered person above age 70 on the date of diagnosis.
Annual Health Screening	Annual benefit of $50 or $100 per year, depending on selection for employee and spouse, for tests and procedures occurring more than sixty (60) days after the Certificate Effective Date.
First Occurrence	First occurrence benefit for the employee is 100% of benefit face amount, 50% for spouse, and 10% for covered children. Each insured person is limited to the payment of only one first occurrence benefit. Reduced to 50% if Insured Person is age 70 or older on the Date of Diagnosis.
Additional Occurrence or Reoccurrence	For each additional occurrence or reoccurrence (after more than 180 days of the last occurrence), a covered critical illness is paid at 50% of the original benefit for employee, spouse, and covered children . Reduced to 50% if Insured Person is age 70 or older on the Date of Diagnosis.
